Melatonin for Shift Workers: Dose, Timing, and What It Won't Fix

Melatonin is the supplement nurses ask about most, and almost everyone takes too much of it at the wrong time. It's a clock signal, not a sedative — which changes both the dose and the hour.

Melatonin is the supplement nurses ask about more than any other, and the way most people take it — 10 mg, swallowed at bedtime because sleep isn’t coming — is close to the opposite of how it works.

It isn’t a sedative. It’s a clock signal. Your pineal gland releases it when light stops hitting your retina, and its job is to tell every other system that biological night has started. Taking it is less like taking a sleeping tablet and more like moving the hands on a clock.

That distinction changes the dose and, more importantly, the hour.

The dose is far smaller than what’s on the shelf

Physiological melatonin output is somewhere in the region of a few tenths of a milligram. Trials of circadian phase shifting have generally found 0.5 to 1 mg does the job, and that higher doses are not better — they raise blood levels well past anything the body produces, keep them elevated into the following day, and are more likely to leave you groggy on shift.

Retail capsules are commonly 5 or 10 mg. That’s five to twenty times the useful amount.

Start at 0.5 mg

If your bottle is 5 mg, splitting a tablet is reasonable; many are scored. This is one of very few supplements where the sensible move is to take substantially less than the packaging suggests.

Timing is the whole thing

Melatonin shifts your body clock in a direction that depends on when you take it. Take it at the wrong point and it either does nothing or moves you further from where you want to be.

For nurses the practical uses are narrow but real:

Sleeping after a night shift. Taken shortly before your daytime sleep, it helps signal night to a body being told by daylight that it’s morning. It reliably improves how quickly you get to sleep. It improves total sleep duration less than people expect — often by only twenty or thirty minutes.

Shifting toward a run of nights. Taken in the evening a day or two before nights start, it can drag your clock later so the first shift is less brutal.

Coming back off nights. Small evening doses can help pull you back onto a day schedule.

What it doesn’t do is get you to sleep at 2200 on a night off after a run of nights. Nothing does, quickly. Your clock is somewhere else and it takes days to move.

It is not a sleeping pill, and treating it as one has a cost

If sleep won’t come and you take another 5 mg at 1100, you’re extending elevated melatonin into the afternoon — and possibly into your drive to work. Grogginess on shift is the common side effect and it’s usually a dosing error rather than the drug.

What the evidence actually supports

The honest summary: modest, real, and narrower than the marketing.

Across shift-work and jet-lag trials melatonin shortens sleep onset latency fairly consistently, produces small increases in total sleep time, and has a good short-term safety profile. Cochrane’s review of melatonin for shift workers found improvements in sleep length that were statistically real and clinically small.

What it doesn’t do:

  • It doesn’t make daytime sleep as restorative as night sleep.
  • It doesn’t improve alertness on shift — that’s a different problem with different answers.
  • It doesn’t offset the health risks associated with long-term night work.

Light exposure, timing, and protecting the sleep before a shift all matter more. Melatonin is a useful addition to that; it is not a substitute for it. Most of what actually helps is in sleeping after nights.

Practical cautions

Supplements aren’t regulated like medicines. US analyses of retail melatonin have repeatedly found actual content varying widely from the label, and some products containing serotonin as a contaminant. Look for third-party verification — USP or NSF — rather than trusting the number on the bottle.

Interactions worth knowing. Melatonin can potentiate sedatives and may interact with anticoagulants and some anticonvulsants. Fluvoxamine substantially raises melatonin levels.

Ask a doctor first if you’re pregnant or breastfeeding, have an autoimmune condition, epilepsy, or take any of the above. Extended-release melatonin is prescription-only in some countries, and if you’re relying on it nightly for months, that’s a conversation rather than a purchase.

The short version

  • 0.5 to 1 mg. Not 5, not 10.
  • Take it immediately before your daytime sleep, not whenever sleep fails.
  • Expect faster sleep onset, not dramatically more sleep.
  • It moves your clock; it does not sedate you.
  • Buy third-party verified, because label accuracy is genuinely poor.
  • It’s an addition to light and timing, not a replacement for them.
